Print this page and fill out both forms. One form for
the starter kit and one form for the removable caps.
2. For any special
decorations, draw on the teeth using the
drawings provided. Be as specific as possible. Use the
Comments section to
give us any addition instructions.
3. After you have completed the form, mail it along with a cashier's check
or money
order to us. You can expect to receive your custom caps in 5-7 business days.
You only need to send one money order or
cashier's check.
